Adrafinil and Aspirin, Carisoprodol, Codeine Phosphate

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES

Why this applies to Aspirin, Carisoprodol, Codeine Phosphate

Aspirin, Carisoprodol, Codeine Phosphate falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Adrafinil interacts with.

Theoretically, adrafinil might increase the levels and clinical effects of CYP2C19 substrates.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to inhibit CYP2C19 activity in vitro.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

This combo pill mixes aspirin, the muscle relaxer carisoprodol, and the opioid codeine. The piece that matters here is carisoprodol (Soma), which your liver turns into meprobamate using an enzyme called CYP2C19. Adrafinil becomes modafinil in your body, and modafinil may mildly slow that same enzyme down.

If carisoprodol clears more slowly, you could feel extra drowsy, dizzy, or foggy, and that stacks on top of the sedation codeine already causes. Honestly though, this is a theoretical concern based on lab data, not strong human studies, so it may not amount to much. Still, layering a stimulant-like supplement on top of two sedating drugs is worth a quick chat with your pharmacist or doctor first.
