Adrafinil and Aspirin, Pentazocine

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

Why this applies to Aspirin, Pentazocine

Aspirin, Pentazocine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Adrafinil interacts with.

Theoretically, adrafinil might reduce the levels and clinical effects of CYP1A2 substrates.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to weakly induce CYP1A2 activity in vitro.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version. Once your liver turns adrafinil into modafinil, that modafinil may gently nudge a liver enzyme called CYP1A2 to work a little faster. This medicine (the aspirin and pentazocine combo in Talwin Compound) is partly handled by that same enzyme. So in theory, a faster enzyme could clear it a bit quicker and slightly soften how well it works, mainly the pentazocine pain relief.

I want to be honest with you though: this is a theoretical concern based on lab data, not real-world reports. Any effect is likely small. The bigger issue is that adrafinil is unregulated and can stress your liver, the very organ doing this work. Please don't start or stop anything on your own. Run it past your pharmacist or doctor first.

From our pharmacist’s overview on the Aspirin, Pentazocine page

“Talwin Compound pairs pentazocine, an opioid painkiller, with aspirin, and I read its supplement list with more concern than most. The great majority of entries do one of two things: raise the bleeding risk that aspirin already carries, or deepen the sedation pentazocine causes.”
