Here's the gist of this pairing. Once adrafinil gets into your body, your liver turns it into modafinil, the actual stimulant. Modafinil is then cleared in part by a liver enzyme called CYP3A4. Erythromycin happens to slow that same enzyme down, so in theory the modafinil could hang around longer and build up. That means you might feel its wakefulness effects more strongly, along with things like jitteriness, a racing heart, trouble sleeping, headache, or anxiety.

I want to be honest that this is a theoretical concern based on how the drugs work, not something proven in studies, so don't panic. Still, adrafinil is unregulated and hard on the liver, so please check with your pharmacist or doctor before combining the two.

Here's the gist for this pairing. Once your body turns adrafinil into modafinil, that modafinil can act as a mild "speed-up" signal for a liver enzyme called CYP3A4. Since erythromycin is partly broken down by that same enzyme, in theory revving it up could clear the antibiotic a little faster and leave somewhat lower levels in your system. The practical worry would be slightly weaker antibiotic coverage, not extra side effects.

I'll be honest with you: this is a theoretical concern based on how these drugs work, not something proven in patients, and modafinil's enzyme effect is described as weak. Still, an antibiotic that isn't fully doing its job is worth avoiding, so please run this combination by your pharmacist or doctor before mixing them.
