Here's the gist for fedratinib (Inrebic). Once your liver turns adrafinil into modafinil, that modafinil may gently block an enzyme called CYP2C19, which is one of the pathways your body uses to clear fedratinib. If that pathway slows down, fedratinib could build up a bit higher than expected.

With a cancer medicine like this, more drug in your system can mean more of its known side effects: nausea, diarrhea, low blood counts, fatigue, or liver enzyme changes. I'll be honest, this is a theoretical concern based mainly on lab data, not real-world reports, so it may not amount to much. But because fedratinib is a serious medication, please loop in your oncologist or pharmacist before pairing it with adrafinil, especially since adrafinil itself can stress the liver.

Here's the concern when you pair adrafinil with fedratinib (Inrebic). Your body turns adrafinil into modafinil, and modafinil can mildly speed up an enzyme in the liver called CYP3A4. Fedratinib is partly cleared by that same enzyme. So in theory, revving it up could lower your fedratinib levels and make it work less well against your myelofibrosis. That matters because fedratinib is a serious cancer medicine, and you want it doing its full job.

I want to be honest that this is theoretical, based on how the drugs behave rather than real patient reports, and modafinil is only a weak inducer. Still, with a cancer drug like this, I wouldn't gamble. Please run it by your oncologist or pharmacist before combining them.

What Is Fedratinib Used For?

Brand name: Inrebic

Inrebic (fedratinib) is approved to treat adult patients with intermediate-2 or high-risk myelofibrosis (MF) — a type of bone marrow cancer that disrupts normal blood cell production. This includes primary myelofibrosis as well as secondary myelofibrosis that developed after polycythemia vera or essential thrombocythemia.

It is used both in patients who have not been treated before and in those who have previously been treated with ruxolitinib, another myelofibrosis medication.
