These two interact in 2 distinct ways. Each one is explained separately below.
Why Are There 2 Interactions?
Adrafinil interacts with whole categories of medication. Guaifenesin, Hydrocodone, Pseudoephedrine falls into 2 of those categories, so it can interact with Adrafinil in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
1 of 2Why this applies to Guaifenesin, Hydrocodone, Pseudoephedrine
Guaifenesin, Hydrocodone, Pseudoephedrine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Adrafinil interacts with.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to weakly induce CYP3A4 activity.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for this combo cough-and-cold product. Once your liver turns adrafinil into modafinil, that modafinil can gently speed up a liver enzyme called CYP3A4. The hydrocodone piece relies partly on that enzyme, so in theory it could be cleared a little faster, which might make it slightly less effective for pain or cough relief. The guaifenesin and pseudoephedrine parts aren't really affected this way.
Honestly, this is a theoretical concern based on how the drugs work, not from strong real-world reports, and modafinil's effect on this enzyme is weak. I wouldn't lose sleep over it, but since adrafinil is unregulated and hard on the liver, please check with your pharmacist or doctor before pairing them.

STIMULANT DRUGS
2 of 2Guaifenesin, Hydrocodone, Pseudoephedrine also falls under STIMULANT DRUGS.
Adrafinil is a precursor to the prescription drug modafinil, which has stimulant effects.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combination cold and cough product contains three different ingredients, and the one that matters here is the pseudoephedrine, the decongestant. Pseudoephedrine is itself a stimulant, and adrafinil turns into modafinil in your liver, which also has wake-promoting, stimulant-like effects. Putting the two together could stack those effects on top of each other.
In real life that might mean a faster or pounding heartbeat, higher blood pressure, jitteriness, trouble sleeping, or feeling wired and anxious. To be honest, this concern is theoretical, based on how these substances work rather than solid studies, so it may not happen to everyone. Still, since adrafinil is unregulated and hard on the liver, please run this by your pharmacist or doctor before mixing them.
