Guanfacine falls under ANTIHYPERTENSIVE DRUGS — the group Adrafinil interacts with.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to increase diastolic and systolic blood pressure. Retrospective analyses show that patients taking modafinil had a small but increased rate of requiring new antihypertensive drugs or increased use of antihypertensive drugs when compared with placebo.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version: guanfacine works to bring your blood pressure (and in some cases ADHD symptoms) down by calming the nervous system, slowing your heart rate, and relaxing your blood vessels. Adrafinil turns into modafinil in your body, and modafinil is a stimulant that can nudge blood pressure up a bit. So the two are essentially pulling in opposite directions.
The concern is that adrafinil could partly work against your guanfacine, meaning your blood pressure may not be as well controlled, or your readings might creep higher. I want to be honest, though: this is a theoretical interaction based on how these drugs behave, not on strong studies in people taking them together. Still, since adrafinil is unregulated and hard on the liver, please check with your doctor or pharmacist before combining them.

What Is Guanfacine Used For?
Brand name: Tenex
Guanfacine immediate-release tablets (Guanfacine, Guanfacine Hydrochloride) are used to manage high blood pressure (hypertension). They can be used alone or together with other blood pressure medicines, especially thiazide-type diuretics (water pills).
Guanfacine extended-release tablets — sold as Guanfacine ER, Guanfacine extended-release, and Intuniv — are approved for the treatment of ADHD in children and adolescents ages 6 to 17. They can be used on their own (monotherapy) or alongside stimulant ADHD medications as add-on therapy.
