Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to increase diastolic and systolic blood pressure. Retrospective analyses show that patients taking modafinil had a small but increased rate of requiring new antihypertensive drugs or increased use of antihypertensive drugs when compared with placebo.

Your Inderide combines two blood pressure medicines: hydrochlorothiazide (a water pill) and propranolol (a beta blocker). They work together to keep your numbers down. Adrafinil is different. Your liver turns it into modafinil, a wakefulness drug that can nudge blood pressure and heart rate up. So you have one product pulling your pressure down and another that may push back against it.

In theory, that tug-of-war could blunt how well your Inderide controls your blood pressure. Keep in mind this is based on pharmacology and small retrospective data, not strong proof, so it may not happen to everyone. Still, if you use adrafinil, check your readings and talk with your pharmacist or doctor before mixing the two.

Theoretically, adrafinil might increase the levels and clinical effects of CYP2C19 substrates.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to inhibit CYP2C19 activity in vitro.

Here's the gist with Inderide, which combines two blood pressure medicines: hydrochlorothiazide (a water pill) and propranolol (a beta-blocker). The piece that matters here is propranolol, which the liver partly clears using an enzyme called CYP2C19. Once your body turns adrafinil into modafinil, that modafinil may gently slow this enzyme down, so propranolol could build up a bit more than usual.

If that happens, you might feel propranolol's effects more strongly: a slower heart rate, lower blood pressure, lightheadedness, or feeling unusually tired. I want to be honest though, this is theoretical, based on lab data rather than real patient reports, so it may not amount to much. Still, since adrafinil is unregulated and rough on the liver, please run it by your doctor or pharmacist first.

Here's the thing with this combo pill: the propranolol part is partly broken down by a liver enzyme called CYP1A2. Adrafinil turns into modafinil in your body, and modafinil may speed that enzyme up a little. In theory, that could clear propranolol out faster and make it work less well, so your blood pressure or heart rate might not be controlled as smoothly. The hydrochlorothiazide (water pill) side isn't really affected this way.

I want to be honest, though: this is only a theoretical concern based on lab data, not real patient reports, so it may not amount to much. Still, since blood pressure meds matter, check with your pharmacist or doctor before pairing these.
