Hydrocodone, Pseudoephedrine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Adrafinil interacts with.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to weakly induce CYP3A4 activity.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with this combo. Adrafinil turns into modafinil in your body, and modafinil can gently speed up a liver enzyme called CYP3A4. Your hydrocodone leans on that same enzyme. In theory, a faster enzyme could clear hydrocodone a little quicker, which might soften its cough-suppressing and pain-relieving effects. The pseudoephedrine part is mostly cleared by the kidneys, so it's less of a concern here.
I want to be honest that this is a theoretical interaction based on how the drugs work, not strong real-world data, and modafinil is only a weak enzyme booster. Still, since both can affect alertness in different directions, it's worth a quick chat with your pharmacist or doctor before pairing them.

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the piece that matters: your combo product contains pseudoephedrine, a decongestant that has mild stimulant qualities. Adrafinil turns into modafinil in your body, which is also a stimulant. Put them together and the worry is that those effects could stack up. You might notice a racing or pounding heart, higher blood pressure, jitteriness, trouble sleeping, or feeling wired and anxious.
To be fair, this is a theoretical concern based on how these drugs work, not on strong studies, so it may never cause you a problem. Still, since adrafinil is unregulated and hard on the liver, it's worth running this pairing past your pharmacist or doctor before combining them, especially if you have heart or blood pressure issues.
