These two interact in 2 distinct ways. Each one is explained separately below.
Why Are There 2 Interactions?
Adrafinil interacts with whole categories of medication. Hydrocodone, Pseudoephedrine, Chlorpheniramine falls into 2 of those categories, so it can interact with Adrafinil in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
1 of 2Why this applies to Hydrocodone, Pseudoephedrine, Chlorpheniramine
Hydrocodone, Pseudoephedrine, Chlorpheniramine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Adrafinil interacts with.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to weakly induce CYP3A4 activity.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with Zutripro, which combines hydrocodone (a pain and cough opioid), pseudoephedrine (a decongestant), and chlorpheniramine (an antihistamine). Once your liver turns adrafinil into modafinil, that modafinil can mildly speed up an enzyme called CYP3A4. Since hydrocodone is partly handled by that enzyme, the theory is your body could clear it a bit faster, which might soften the cough relief and pain control you're counting on.
I want to keep this in perspective: this is a theoretical concern based on pharmacology, not something well documented in real patients, and the effect is considered weak. Still, since adrafinil is unregulated and rough on the liver, please loop in your pharmacist or doctor before combining them so they can keep an eye on how well the medicine is working.

STIMULANT DRUGS
2 of 2Hydrocodone, Pseudoephedrine, Chlorpheniramine also falls under STIMULANT DRUGS.
Adrafinil is a precursor to the prescription drug modafinil, which has stimulant effects.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the piece to pay attention to in this combo medicine: the pseudoephedrine part is a stimulant decongestant. Adrafinil turns into modafinil in your liver, and modafinil is also a stimulant. Put them together and you may stack stimulant effects on top of each other. In real life that could mean a faster or pounding heartbeat, higher blood pressure, jitteriness, trouble sleeping, and feeling wired or anxious. This is a theoretical concern based on how the drugs work, not strong human studies, so the severity is moderate, not severe.
One more note: the hydrocodone and chlorpheniramine pieces can make you drowsy, so mixing in a stimulant gets unpredictable. Please run this by your pharmacist or doctor before combining them.
