Ozanimod Hydrochloride falls under MONOAMINE OXIDASE INHIBITORS (MAOIs) — the group Adrafinil interacts with.

Theoretically, concomitant use might increase the risk for serious adverse effects.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil, and a related drug, armodafinil, have been associated with reports of serious adverse effects, such as hypertensive crisis and hyperthermia, when used in combination with MAOIs.

Here is the thing to know about Zeposia (ozanimod): one of its active breakdown products actually blocks an enzyme called MAO-B, so it has mild MAO-inhibiting activity in the body. That is why it lands in the MAOI category for interaction checks. Adrafinil, on the other hand, turns into modafinil in your liver, and modafinil is a stimulant-type drug.

When a stimulant like modafinil meets something with MAOI activity, there is a theoretical concern that blood pressure and body temperature could spike (think dangerously high blood pressure or overheating). I want to be honest that this warning is based on pharmacology and caution, not on real-world reports with this exact pair. Still, since Zeposia is a serious MS and colitis medicine, please run any adrafinil use past your doctor or pharmacist first.
