Here's how these two could bump into each other. Adrafinil only works after your liver turns it into modafinil, and that conversion leans on an enzyme called CYP3A4. Pitolisant (Wakix) can mildly slow that same enzyme down. In theory, that could let modafinil build up a bit more than expected, which might mean stronger stimulant effects like trouble sleeping, jitteriness, a faster or pounding heartbeat, headache, or anxiety.

I'll be honest with you though: this is a theoretical concern based on how the drugs are processed, not something proven in real patients. The bigger picture is that both of these promote wakefulness, so stacking them could overdo it. Adrafinil is also unregulated and hard on the liver. Please loop in your doctor or pharmacist before combining them.

Here's the thing to know about pairing these two. Your body turns adrafinil into modafinil, and modafinil can gently speed up a liver enzyme called CYP3A4. Pitolisant (Wakix) happens to be broken down by that same enzyme. So in theory, taking adrafinil could nudge your liver to clear pitolisant a little faster, which might leave you with less pitolisant in your system than expected.

Practically, that could mean your Wakix doesn't control daytime sleepiness or cataplexy as well as it usually does. I'll be honest though, this is a theoretical concern based on how the drugs work, not something proven in studies, and modafinil's effect on this enzyme is fairly weak. Since both drugs target wakefulness anyway, it's worth a quick chat with your doctor or pharmacist before combining them.

What Is Pitolisant Used For?

Brand name: Wakix

Wakix (pitolisant) is used to treat two hallmark problems of narcolepsy: excessive daytime sleepiness (feeling overwhelmingly tired and unable to stay awake during the day) and cataplexy (sudden episodes of muscle weakness or loss of muscle control, often triggered by emotion). It is approved for adults and children 6 years of age and older.

Both uses are specific to Wakix tablets taken by mouth. The drug is not approved for sleepiness or cataplexy caused by conditions other than narcolepsy.
