Risperidone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Adrafinil interacts with.

Theoretically, adrafanil might reduce levels and clinical effects of CYP3A4 substrates.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to weakly induce CYP3A4 activity.

Here's the gist: once your liver turns adrafinil into modafinil, that modafinil can mildly speed up an enzyme called CYP3A4. Since your body uses CYP3A4 (along with another enzyme) to clear risperidone, faster clearance could nudge your risperidone levels down a bit. In plain terms, your medication might not work quite as well, so symptoms it controls (like agitation, mood swings, or disturbed thinking) could creep back.

I want to keep this in perspective. This is a theoretical concern based on how the drugs behave, not from strong studies, and modafinil is only a weak booster of that enzyme. Still, adrafinil is unregulated and hard on the liver. Please loop in your doctor or pharmacist before combining the two so they can watch how you're doing.

What Is Risperidone Used For?

Brand names: Risperdal, Perseris

Risperidone is prescribed for three main conditions: schizophrenia (a disorder affecting how a person thinks, feels, and perceives reality), acute manic or mixed episodes in Bipolar I Disorder, and irritability associated with autistic disorder — including aggression, self-injury, temper tantrums, and rapid mood swings.

For bipolar mania, it can be used alone or alongside lithium or valproate. It is approved for use in both adults and certain age groups of children and adolescents, depending on the condition being treated.
