Here the concern runs from ticagrelor toward adrafinil, not the other way around. Adrafinil turns into modafinil in your body, and modafinil is cleared by a liver enzyme called CYP3A4. Ticagrelor can mildly block that same enzyme, so in theory modafinil could clear a little slower and build up. That might mean more of modafinil's stimulant effects, like trouble sleeping, a racing or pounding heart, jitteriness, headache, or higher blood pressure.

I want to be honest that this is theoretical, based on how these drugs are processed rather than real-world reports, so I wouldn't panic. But ticagrelor is a serious heart medication, and adrafinil is unregulated and can stress the liver. Please run this combination by your doctor or pharmacist before mixing them.

Here's the gist for Brilinta (ticagrelor). Your liver uses an enzyme called CYP3A4 to process ticagrelor, and that same enzyme actually helps turn it into one of its active forms. Adrafinil becomes modafinil in your body, and modafinil can mildly speed up (induce) CYP3A4. In theory, a faster enzyme could lower your ticagrelor levels, which might mean it doesn't thin your blood as well as it should.

Why that matters: ticagrelor is keeping clots from forming after a heart attack, stent, or stroke, so you really don't want it working at less than full strength. Now, this concern is theoretical and based on how the drugs work, not on solid studies, so don't panic. Just talk with your pharmacist or doctor before pairing these, and don't change your Brilinta on your own.

About Ticagrelor

What Is Ticagrelor Used For?

Brand name: Brilinta

Ticagrelor tablets are used to lower the risk of cardiovascular death, heart attack, and stroke in adults who have had an acute coronary syndrome (ACS — a sudden reduction of blood flow to the heart) or a previous heart attack. They also reduce the risk of stent thrombosis (a blood clot forming in a coronary stent) in people stented during ACS treatment.

Ticagrelor tablets are also used to reduce the risk of a first heart attack or stroke in people with coronary artery disease (CAD) who are at high risk — including those with type 2 diabetes. Brilinta is additionally indicated to reduce the risk of stroke in adults with a mild acute ischemic stroke or a high-risk transient ischemic attack (TIA, sometimes called a "mini-stroke").
