Trabectedin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Adrafinil interacts with.

Theoretically, adrafanil might reduce levels and clinical effects of CYP3A4 substrates.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to weakly induce CYP3A4 activity.

Here's the gist for Yondelis (trabectedin). Your liver uses an enzyme called CYP3A4 to process this chemo drug. Once your body turns adrafinil into modafinil, that modafinil can mildly speed up CYP3A4. In theory, that faster clearing could lower the amount of trabectedin in your system, which might make it work less well against the sarcoma.

I want to be honest about how strong this is: it's a theoretical concern based on how these drugs behave, not something proven in patients, and modafinil is only a weak inducer. Still, trabectedin is serious chemotherapy where dose really matters, so please don't combine these without checking with your oncology team or pharmacist first.

What Is Trabectedin Injection Used For?

Brand name: Yondelis

Yondelis (trabectedin injection) is used to treat adults with liposarcoma or leiomyosarcoma — two types of soft tissue sarcoma (cancer of connective tissue) — when the cancer cannot be removed by surgery (unresectable) or has spread to other parts of the body (metastatic). It is indicated for patients who have already received a prior treatment regimen containing an anthracycline (a type of chemotherapy).
