Aspirin, Caffeine, Codeine Phosphate falls under CNS DEPRESSANTS — the group Ashwagandha interacts with.
Ashwagandha seems to have sedative effects. Theoretically, this may potentiate the effects of barbiturates, other sedatives, and anxiolytics.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combination product contains codeine, an opioid that can make you drowsy. Ashwagandha is thought to have its own mild calming, sleepy effect. Put the two together and you may simply feel more tired, foggy, or relaxed than you would expect from the medicine alone. Caffeine in the mix can offset some of that, but it doesn't cancel out the codeine.
I'll be honest: this concern is mostly theoretical, based on how each acts in the body rather than strong human studies, so don't panic. Still, until you know how you react, be careful with driving or anything that needs full focus, and skip alcohol. If you take this pain reliever regularly and want to try ashwagandha, run it past your pharmacist or doctor first.

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

In vitro research shows that ashwagandha extract induces CYP1A2 enzymes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
In this combination product, the piece that interacts with ashwagandha is the caffeine. Caffeine is cleared by an enzyme called CYP1A2, and lab studies hint that ashwagandha may speed that enzyme up. If that happens, your body could clear caffeine a little faster, which might mean slightly less of caffeine's usual lift (a touch less alertness or a weaker pep). The aspirin and codeine parts are not really affected here.
Honestly, this is a very weak, theoretical concern based only on test-tube research, so I wouldn't lose sleep over it. The effect, if any, is mild. Still, if you take ashwagandha regularly and notice changes, just check in with your pharmacist or doctor.

Here's the gist for this combo pill. Lab studies hint that ashwagandha may gently speed up a liver enzyme called CYP3A4, which helps process the codeine portion of this medicine. If that happens, your body might clear the codeine a little faster, which could mean slightly less pain relief or a weaker effect than you'd expect. The aspirin and caffeine pieces aren't really affected here.
I'll be honest with you though: this is only a theoretical concern based on test-tube research, not real patients, and it's rated as minor. For most people taking ashwagandha short-term, it's unlikely to cause a noticeable problem. Still, if your pain isn't well controlled, mention the ashwagandha to your pharmacist or doctor.
