Ashwagandha and Aspirin, Caffeine, Hydrocodone

These two interact in 3 distinct ways. Each one is explained separately below.

Why Are There 3 Interactions?

Ashwagandha interacts with whole categories of medication. Aspirin, Caffeine, Hydrocodone falls into 3 of those categories, so it can interact with Ashwagandha in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

CNS DEPRESSANTS

1 of 3

Why this applies to Aspirin, Caffeine, Hydrocodone

Aspirin, Caffeine, Hydrocodone falls under CNS DEPRESSANTS — the group Ashwagandha interacts with.

Theoretically, taking ashwagandha might increase the sedative effects of CNS depressants.
Ashwagandha seems to have sedative effects. Theoretically, this may potentiate the effects of barbiturates, other sedatives, and anxiolytics.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

This combo product contains hydrocodone, an opioid pain reliever that can make you drowsy and slow your breathing on its own. Ashwagandha appears to have mild calming, sedative-like effects of its own. So the worry here is simple: stacking the two together could add up and leave you more sleepy, foggy, or sluggish than you'd expect, especially when you first start.

I'll be honest, this concern is theoretical, based on how the herb seems to behave rather than solid human studies, so for most people it's not a big alarm. Still, if you notice extra drowsiness, dizziness, or trouble concentrating, that's worth taking seriously. The caffeine in the product won't reliably cancel it out. Please check with your pharmacist or doctor before pairing these.

Aspirin, Caffeine, Hydrocodone also falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES.

Theoretically, ashwagandha might decrease the levels and clinical effects of CYP1A2 substrates.
In vitro research shows that ashwagandha extract induces CYP1A2 enzymes.

Dr. Brian Staiger, PharmD, BCPS

This combination pill mixes aspirin, caffeine, and hydrocodone. The piece that overlaps with ashwagandha here is the caffeine, which your body clears using an enzyme called CYP1A2. Lab studies hint that ashwagandha may speed that enzyme up, so in theory your body could break caffeine down a little faster and you might feel less of its perk-up effect. Honestly, this is a small, theoretical concern based only on test-tube and animal data, not real patient cases.

I wouldn't lose sleep over it. The aspirin and hydrocodone parts aren't really affected by this particular pathway. Still, if you take ashwagandha regularly, mention it to your pharmacist or doctor so they can keep an eye on things and confirm it fits the rest of your routine.

This combo pill blends aspirin (a pain reliever), caffeine (a mild stimulant), and hydrocodone (an opioid for pain). The piece that touches this interaction is hydrocodone, which the liver partly handles using an enzyme called CYP3A4. Lab studies hint that ashwagandha might speed that enzyme up. In theory, that could clear hydrocodone a little faster and make your pain relief feel slightly weaker.

Honestly, this is a very soft signal. It comes only from test-tube research, the rating is minor, and we don't have human studies showing it actually matters. I wouldn't lose sleep over it, but it's still worth a quick chat with your pharmacist or doctor, especially if you notice your pain isn't as well controlled.
