Why this applies to Sacubitril, Valsartan
Sacubitril, Valsartan falls under ANTIHYPERTENSIVE DRUGS — the group Ashwagandha interacts with.
Animal research suggests that ashwagandha might lower systolic and diastolic blood pressure. Theoretically, ashwagandha might have additive effects when used with antihypertensive drugs and increase the risk of hypotension.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist: Entresto (sacubitril/valsartan) already does a good job of relaxing your blood vessels and easing the workload on your heart, which naturally lowers your blood pressure. Ashwagandha may nudge blood pressure down a little too. Put them together and those effects could stack up, so your pressure might dip lower than expected. That could leave you feeling dizzy, lightheaded, tired, or wobbly when you stand up quickly.
One honest caveat: this concern comes mostly from animal studies, not solid human trials, so it's more of a theoretical heads-up than a proven problem. Still, if you'd like to try ashwagandha, check with your pharmacist or doctor first, and keep an eye on your blood pressure readings. Please don't change your Entresto dose on your own.

What Is Valsartan and Sacubitril Used For?
Brand name: Entresto
Valsartan and sacubitril is used for heart failure. In adults with chronic heart failure, it lowers the risk of cardiovascular death and hospitalization for heart failure. Benefits are most clearly seen in people whose heart pumps less blood than normal (a below-normal left ventricular ejection fraction, or LVEF).
It is also used to treat symptomatic heart failure with left ventricular systolic dysfunction (the main pumping chamber not squeezing well) in children aged one year and older. This use is listed for Entresto and for the sacubitril and valsartan tablet products.
