Here's the thing to know with Zileuton: it's one of those asthma medications that can be tough on the liver. That's actually why your doctor checks your liver enzymes with blood tests, especially during your first few months on it. Zileuton can raise those liver numbers on its own.

Ashwagandha has been tied to rare but serious liver problems in some reports, so the worry is simple addition. Stacking two things that stress the liver could, in theory, raise your risk together. This is a theoretical concern based on how each behaves, not proven cases of the two combined, so please don't panic. Still, watch for yellowing skin or eyes, dark urine, belly pain, or unusual tiredness, and check with your pharmacist or doctor before adding ashwagandha.

Here's the gist with Zileuton (Zyflo) and ashwagandha. Your body clears Zileuton partly through a liver enzyme called CYP1A2. In lab studies, ashwagandha appears to speed up that enzyme. If that happened in real life, your body might break down Zileuton a little faster, which could mean slightly less protection against your asthma symptoms (more wheezing, coughing, chest tightness, or shortness of breath over time). Zileuton is a regular active drug, not a prodrug, so faster clearing means weaker effect, not stronger.

The good news: this is only theory from test-tube research, rated minor, so it may not matter much in practice. Still, since steady asthma control is important, mention ashwagandha to your pharmacist or doctor before starting it, and watch how your breathing feels.

Here's the gist for Zileuton and ashwagandha. Zileuton is partly cleared by a liver enzyme called CYP3A4. In lab dish studies, ashwagandha appeared to speed that enzyme up. If that happened in real life, your body might break down Zileuton a little faster, which could leave less of it working to keep your airways calm. In plain terms, your asthma control could slip slightly, meaning more wheezing, coughing, or chest tightness.

About Zileuton

What Is Zileuton Used For?

Brand names: Zyflo, Zyflo CR

Zileuton is used for the ongoing prevention and treatment of asthma in adults and adolescents aged 12 and older. It is a maintenance medicine — meaning it is taken regularly to keep asthma under control, not to stop an asthma attack once it has started.

Zyflo (the tablet form) and zileuton extended-release tablets are both approved for this same purpose. Neither form should be used to treat a sudden asthma attack. However, both can be continued during a flare-up of asthma while you also use your rescue inhaler.
