SEROTONERGIC DRUGS

Methadone falls under SEROTONERGIC DRUGS — the group Ayahuasca interacts with.
In vitro and animal research shows that harmaline and harmine, constituents of ayahuasca, strongly and reversibly inhibit monoamine oxidase A and weakly inhibit monoamine oxidase B. The dimethyltryptamine and tetrahydroharmine constituents of ayahuasca have also demonstrated serotonergic activity. A case of serotonin syndrome characterized by tremors, sweating, shivering, confusion, nausea, vomiting, and disorientation has been reported in a 36-year-old male who consumed 100 mL of ayahuasca while taking fluoxetine 20 mg daily. Combining serotonergic drugs with ayahuasca might increase the risk of serotonergic side effects including serotonin syndrome and cerebral vasoconstrictive disorders.

Here's the thing many folks don't realize about methadone: beyond easing pain and preventing withdrawal, it also nudges up serotonin in your brain. Ayahuasca does something similar. Its vine compounds (harmine and harmaline) act like MAO inhibitors, and the DMT it carries is serotonin-active too. Stacking the two could pile too much serotonin together.
When serotonin climbs too high, you can land in serotonin syndrome, with shaking, sweating, shivering, a racing heart, confusion, nausea, and agitation. This is mostly theoretical for methadone (based on how these chemicals behave, not solid human studies), but a real case happened with a serotonin antidepressant plus ayahuasca. Please don't change your methadone, and talk with your doctor or pharmacist before going anywhere near ayahuasca.

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

Ayahuasca contains beta-carboline alkaloids, including harmine and harmaline. In vitro research suggests that the beta-carboline alkaloids might inhibit CYP2D6 enzymes. This effect has not been reported in humans.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the simple version for methadone. Lab studies hint that the harmine and harmaline in ayahuasca may slow down a liver enzyme called CYP2D6, which is one of the routes your body uses to clear methadone. If that pathway is blocked, methadone could build up a bit more than usual. Methadone is an active drug (not a prodrug), so more of it in your system means stronger opioid effects, like heavy drowsiness, slowed breathing, low blood pressure, or confusion.
I want to be honest, though: this is based only on test-tube data and hasn't been shown to happen in real people, so it's theoretical. Even so, methadone is a touchy medication, and ayahuasca's MAOI effects bring their own serious risks. Please don't combine them without talking to your doctor or pharmacist first.

Ayahuasca contains beta-carboline alkaloids. In vitro research suggests that beta-carboline alkaloids competitively inhibit CYP3A4 enzymes. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for methadone: your liver uses an enzyme called CYP3A4 to help clear it out. Lab studies suggest the beta-carboline compounds in ayahuasca can slow that enzyme down. If that happened in the body, methadone could build up a bit higher than usual, which might mean more sedation, slowed or shallow breathing, and a longer QT effect on the heart. Methadone is already a drug we watch carefully, so even modest changes deserve attention.
That said, this is theoretical, based only on test-tube work, not real patients. On top of that, ayahuasca is a strong MAOI and mixing it with opioids carries its own serious risks. Please talk with your doctor or pharmacist before combining anything here.

What Is Methadone Used For?

Methadone treats two main problems. Some products are used for severe, persistent pain that needs daily, around-the-clock opioid treatment when other options are not adequate. Some products are used for opioid addiction (heroin or other morphine-like drugs), as detoxification or as maintenance treatment along with social and medical services.
Not every product is approved for every use. Methadone tablets, oral solution and oral concentrate (Intensol) list both pain and opioid addiction. Diskets, Methadose Dispersible tablets and the Methadose and Methadone Hydrocloride oral concentrates are labeled only for opioid addiction. Methadone injection is for severe pain, and for short-term treatment of opioid dependence in people who cannot take medicine by mouth, such as hospitalized patients. It is not approved for outpatient opioid dependence treatment. Methadone is not meant to be used as an as-needed pain reliever.
