Research on the effects of bacopa extracts on CYP1A2 enzymes is conflicting. Some in vitro evidence shows that bacopa extract can moderately and non-competitively inhibit CYP1A2, while other in vitro evidence suggests that any effect is unlikely to be clinically significant.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the honest picture with this cough and cold combo (the Vicks Formula 44M product that pairs acetaminophen, chlorpheniramine, and dextromethorphan). In a test tube, bacopa has shown a mild ability to slow down a liver enzyme called CYP1A2. If that happened in your body, the ingredients cleared partly through that pathway could hang around a bit longer, which in theory might mean a little more drowsiness or grogginess from the chlorpheniramine, or more of dextromethorphan's effects like dizziness.
That said, the lab evidence is mixed, and none of these are prodrugs that bacopa would weaken. This is a theoretical concern, not something proven in people. Still, since this medicine already causes sleepiness, it's worth a quick word with your pharmacist before combining them.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Research on the effects of bacopa extracts on CYP3A4 enzymes is conflicting. Some in vitro evidence suggests that bacopa extract can moderately and competitively inhibit CYP3A4, while other in vitro evidence suggests that any effect is unlikely to be clinically significant.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combo cold product (the acetaminophen, chlorpheniramine, and dextromethorphan in Vicks Formula 44M) is partly handled by a liver enzyme called CYP3A4. In test-tube studies, bacopa might mildly slow that enzyme down. If that happened in the body, a little more of the dextromethorphan portion could linger, which could mean extra drowsiness or dizziness on top of the sleepiness chlorpheniramine already causes.
Here's the reassuring part: this is only theoretical, based on lab studies that actually disagree with each other, so a real-world problem is unlikely. None of these are prodrugs that bacopa would weaken. Still, since both can make you sleepy, go easy and check with your pharmacist if you take bacopa regularly.

ANTICHOLINERGIC DRUGS
3 of 3Acetaminophen, Chlorpheniramine Maleate, Dextromethorphan HBr also falls under ANTICHOLINERGIC DRUGS.
Bacopa seems to inhibit acetylcholinesterase and might increase acetylcholine levels, which could counteract the effects of anticholinergic drugs. Similarly, anticholinergic drugs might counteract the cholinergic effects of bacopa.

The piece of this combo product that matters here is the chlorpheniramine, an antihistamine that works by blocking acetylcholine, one of the body's chemical messengers. Bacopa is thought to do the opposite, nudging acetylcholine levels up a bit. In theory, the two could partly cancel each other out, so the chlorpheniramine might not control your runny nose or sneezing quite as well, and bacopa's effects could be blunted too. The acetaminophen (pain and fever) and dextromethorphan (cough) parts aren't really involved.
Honestly, this is a theoretical concern based on how each works, not real-world reports, so I wouldn't lose sleep over it. Still, if you take bacopa regularly, mention it to your pharmacist or doctor so they can keep an eye on things.
