Why this applies to Aliskiren

Aliskiren falls under ANTIHYPERTENSIVE DRUGS — the group Breadfruit interacts with.

Theoretically, concomitant use with antihypertensive drugs might increase the risk of hypotension. Animal research shows that breadfruit leaf extract can decrease heart rate and weaken the force of heart muscle contractions, which may lower blood pressure.
Some antihypertensive drugs include captopril (Capoten), enalapril (Vasotec), losartan (Cozaar), valsartan (Diovan), diltiazem (Cardizem), amlodipine (Norvasc), hydrochlorothiazide (HydroDIURIL), furosemide (Lasix), and many others.

Here's the gist: Aliskiren (Tekturna) already lowers your blood pressure by relaxing your blood vessels. Concentrated breadfruit leaf products may nudge blood pressure down too. In animal studies, breadfruit leaf extract slowed the heart rate and softened how hard the heart squeezes, which can drop pressure further. Stack that on top of aliskiren and the two effects could add up.

If your pressure dips too low, you might feel dizzy, lightheaded, tired, or woozy when you stand up. I want to be honest though: this concern is theoretical, based on lab and animal work, not human trials. Eating breadfruit as a food is very unlikely to matter. Concentrated leaf teas or extracts are the bigger question, so check with your pharmacist or doctor before using them.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Aliskiren also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, breadfruit might inhibit cytochrome P450 3A4 (CYP3A4) and increase levels of drugs metabolized by these enzymes; until more is known, use with caution. In vitro, the breadfruit constituent artocarpin can inhibit CYP3A4 enzymes. Also, animal research shows that breadfruit leaf extract can weakly inhibit CYP3A4 enzymes. However, it is unclear if this inhibitory activity causes a clinically meaningful effect on the metabolism of CYP3A4 substrates.
Some drugs metabolized by CYP3A4 include amitriptyline (Elavil), amiodarone (Cordarone), citalopram (Celexa), felodipine (Plendil), lansoprazole (Prevacid), ondansetron (Zofran), prednisone (Deltasone), sertraline (Zoloft), and many others.

Here's the gist for aliskiren (Tekturna). Your body partly clears this blood pressure medicine using an enzyme called CYP3A4. Lab and animal studies suggest a compound in breadfruit leaves may gently slow that enzyme down. In theory, slowing it could let a bit more aliskiren linger in your system, which might nudge its effect a little stronger. That could mean blood pressure dropping more than expected, with possible dizziness, lightheadedness, or feeling tired.

That said, this is very weak, theory-only evidence, and it isn't clear it matters in real people. Eating breadfruit as a food is unlikely to cause trouble. The bigger question is concentrated leaf teas or extracts. If you're using those, please check with your pharmacist or doctor first.

What Is Aliskiren Used For?

Brand name: Tekturna

Aliskiren tablets — available as the brand Tekturna and as generic aliskiren — are used to treat high blood pressure (hypertension) in adults and in children who are at least 6 years old and weigh 50 kg (about 110 lbs) or more. Lowering blood pressure reduces the risk of strokes and heart attacks.

Controlling blood pressure is most effective as part of a broader plan that may include healthy eating, exercise, reducing sodium, and managing other risk factors like cholesterol and blood sugar. Many people need more than one blood pressure medicine to reach their goal.
