Why this applies to Atazanavir

Atazanavir falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Breadfruit interacts with.

Theoretically, breadfruit might inhibit cytochrome P450 3A4 (CYP3A4) and increase levels of drugs metabolized by these enzymes; until more is known, use with caution. In vitro, the breadfruit constituent artocarpin can inhibit CYP3A4 enzymes. Also, animal research shows that breadfruit leaf extract can weakly inhibit CYP3A4 enzymes. However, it is unclear if this inhibitory activity causes a clinically meaningful effect on the metabolism of CYP3A4 substrates.
Some drugs metabolized by CYP3A4 include amitriptyline (Elavil), amiodarone (Cordarone), citalopram (Celexa), felodipine (Plendil), lansoprazole (Prevacid), ondansetron (Zofran), prednisone (Deltasone), sertraline (Zoloft), and many others.

Here's the gist: your body relies on a liver enzyme called CYP3A4 to break down atazanavir. Lab and animal studies hint that compounds in breadfruit leaf may slow that enzyme down a little. If that happened in people, atazanavir could clear more slowly and its levels could creep up, which might mean more of its typical side effects like yellowing of the skin or eyes (jaundice), nausea, or an irregular heartbeat.

The good news is that this is only a theoretical, weak signal from test tubes, not from people. Eating breadfruit as a food is very unlikely to matter. Concentrated leaf teas or extracts are the bigger unknown, so since atazanavir is a key HIV medicine, please check with your pharmacist or doctor before using them.

About Atazanavir

What Is Atazanavir Used For?

Brand name: Reyataz

Atazanavir capsules are used together with other antiretroviral medicines to treat HIV-1 infection in adults and in children 6 years and older who weigh at least 15 kg (about 33 lbs).

Reyataz — available as capsules and as an oral powder — is also approved for HIV-1 treatment in adults and in children as young as 3 months who weigh at least 5 kg (about 11 lbs). Reyataz oral powder must always be taken with ritonavir and food, and is not for use in children weighing less than 5 kg.
