Codeine Phosphate, Guaifenesin falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Cajeput Oil interacts with.

Evidence from in vitro research suggests that cajeput leaf extract can inhibit cytochrome P450 2D6 (CYP2D6) by approximately 30%. Theoretically, cajeput oil may increase levels of drugs metabolized by CYP2D6. Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), clozapine (Clozaril), codeine, desipramine (Norpramin), donepezil (Aricept), fentanyl (Duragesic), flecainide (Tambocor), fluoxetine (Prozac), meperidine (Demerol), methadone (Dolophine), metoprolol (Lopressor, Toprol XL), olanzapine (Zyprexa), ondansetron (Zofran), tramadol (Ultram), trazodone (Desyrel), and others.

Here's the wrinkle with codeine: it's actually a prodrug. Your body has to convert it using an enzyme called CYP2D6 into morphine, which is what does most of the pain and cough relief. The guaifenesin part just loosens mucus and isn't really involved here.

Lab research hints that cajeput oil might slow CYP2D6 by roughly 30%. If that happened in real life, it could mean less codeine gets turned into morphine, so you might get weaker pain or cough relief rather than more. Keep in mind this is theoretical, based only on test-tube work, not people, and essential oils shouldn't be swallowed anyway. If you use cajeput oil and feel codeine isn't working as expected, check in with your pharmacist or doctor.
