Donepezil falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Cajeput Oil interacts with.

Evidence from in vitro research suggests that cajeput leaf extract can inhibit cytochrome P450 2D6 (CYP2D6) by approximately 30%. Theoretically, cajeput oil may increase levels of drugs metabolized by CYP2D6. Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), clozapine (Clozaril), codeine, desipramine (Norpramin), donepezil (Aricept), fentanyl (Duragesic), flecainide (Tambocor), fluoxetine (Prozac), meperidine (Demerol), methadone (Dolophine), metoprolol (Lopressor, Toprol XL), olanzapine (Zyprexa), ondansetron (Zofran), tramadol (Ultram), trazodone (Desyrel), and others.

Here's the gist for donepezil (Aricept): your body partly clears this medicine using a liver enzyme called CYP2D6. In a lab dish, compounds in cajeput oil slowed that enzyme by roughly 30 percent. If that held true in people, your donepezil could clear a little slower and build up somewhat, which might nudge up its usual side effects like nausea, diarrhea, dizziness, a slow heartbeat, trouble sleeping, or muscle cramps.

I want to be honest with you though: this is only a theoretical concern based on test-tube work, not real patient cases, so it may not amount to much in practice. Still, since donepezil already affects heart rate and the gut, it's worth a quick chat with your pharmacist or doctor before using cajeput oil regularly. Please don't change your donepezil on your own.
