Mexiletine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Cajeput Oil interacts with.

Evidence from in vitro research suggests that cajeput leaf extract can inhibit cytochrome P450 2D6 (CYP2D6) by approximately 30%. Theoretically, cajeput oil may increase levels of drugs metabolized by CYP2D6. Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), clozapine (Clozaril), codeine, desipramine (Norpramin), donepezil (Aricept), fentanyl (Duragesic), flecainide (Tambocor), fluoxetine (Prozac), meperidine (Demerol), methadone (Dolophine), metoprolol (Lopressor, Toprol XL), olanzapine (Zyprexa), ondansetron (Zofran), tramadol (Ultram), trazodone (Desyrel), and others.

Here's the gist with mexiletine and cajeput oil. Your body relies on a liver enzyme called CYP2D6 to clear mexiletine out. Lab studies hint that cajeput extract can slow that enzyme down by about a third. If that holds true in people, mexiletine could build up a bit more than expected. Mexiletine is active as-is (not a prodrug), so higher levels mean stronger effects and a greater chance of side effects like nausea, dizziness, tremor, blurred vision, or, more importantly, new heart rhythm problems.

Now, the honest part: this is based only on test-tube data, not real patients, so it's theoretical. Still, because mexiletine is a heart rhythm drug where small shifts matter, I'd run any cajeput oil use past your pharmacist or doctor first.

About Mexiletine

What Is Mexiletine Used For?

Brand name: Mexitil

Mexiletine Hydrochloride capsules are indicated for the treatment of documented, life-threatening ventricular arrhythmias — for example, sustained ventricular tachycardia (a dangerously fast heart rhythm originating in the lower chambers). Because mexiletine itself can trigger arrhythmias, it is not recommended for minor or symptom-free irregular heartbeats.

Treatment is started in a hospital setting so heart rhythm can be monitored closely. Antiarrhythmic drugs, including mexiletine, have not been shown to improve overall survival in patients with ventricular arrhythmias.
