Felodipine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cascara Sagrada interacts with.

Theoretically, cascara sagrada might decrease the effects of CYP3A4 substrates.
In vitro research suggests that cascara sagrada can induce CYP3A4 enzymes, albeit to a much lower degree than rifampin, a known CYP3A4 inducer.

Here's the gist for these two together. Felodipine is cleared from your body by a liver enzyme called CYP3A4. Lab studies hint that cascara sagrada might gently speed up that enzyme, which could in theory let your body clear felodipine a little faster. If that happened, your blood pressure medicine might not work quite as well, and you could see your numbers creep up.

That said, I wouldn't lose sleep over this one. The evidence is only from test-tube and lab work, the effect was described as much weaker than a strong inducer like rifampin, and the rating is minor. The bigger real-world concern with cascara is cramping, dehydration, and electrolyte loss from short-term laxative use. If you plan to use cascara, give your pharmacist or doctor a quick heads-up first.
