Here's the gist for Entresto (sacubitril/valsartan). The valsartan piece can nudge your potassium upward, so on its own it doesn't usually cause low potassium. Cascara sagrada works the other direction: as a stimulant laxative, using it often or for too long can flush out potassium and leave you low. So the real worry isn't a clash between the two drugs, it's that heavy or prolonged cascara use could throw off your potassium and fluid balance while your heart medication is also working on those same systems.

Keep in mind this is a theoretical concern based on how these work, not strong proof. Still, with heart failure, swings in potassium, dehydration, dizziness, or muscle cramps matter. Please check with your pharmacist or doctor before using cascara.

Cascara sagrada is a stimulant laxative made from the aged bark of a Pacific Northwest tree, used mainly for short-term relief of constipation. Because it can cause cramping, dehydration, and electrolyte problems—and was removed from over-the-counter laxative products in the U.S. due to safety concerns—it should only be used briefly, if at all, and with guidance from a pharmacist or doctor.

What Is Valsartan and Sacubitril Used For?

Brand name: Entresto

Sacubitril and valsartan is used to reduce the risk of cardiovascular death and hospitalization for heart failure in adult patients with chronic heart failure. The benefit is most clearly seen in patients whose heart pumps with a lower-than-normal ejection fraction (the percentage of blood the heart pushes out with each beat).

It is also approved to treat symptomatic heart failure with reduced left ventricular function in children aged 1 year and older. In pediatric patients, it lowers a blood marker of heart stress (NT-proBNP) and is expected to improve heart outcomes.
