Why this applies to Codeine, Paracetamol

Codeine, Paracetamol falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2D6 (CYP2D6) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2D6. Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), fluoxetine (Prozac), ondansetron (Zofran), tramadol (Ultram), and others.

Here's the wrinkle with Co-codamol: codeine is what we call a prodrug. On its own it does very little. Your body has to use a liver enzyme called CYP2D6 to turn codeine into morphine, which is what actually relieves pain. A lab study found that plumbagin (a compound in Ceylon Leadwort) can block CYP2D6. If that happened in real life, less codeine would convert to morphine, so you might get weaker pain relief from the codeine part. The paracetamol portion isn't affected this way.

The important caveat: this is only test-tube evidence and has never been shown in people, so I wouldn't lose sleep over it. Still, if you plan to use this supplement, run it by your pharmacist or doctor first.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 1A2 (CYP1A2) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP1A2. Some substrates of CYP1A2 include clozapine (Clozaril), cyclobenzaprine (Flexeril), fluvoxamine (Luvox), haloperidol (Haldol), imipramine (Tofranil), mexiletine (Mexitil), olanzapine (Zyprexa), pentazocine (Talwin), propranolol (Inderal), tacrine (Cognex), zileuton (Zyflo), zolmitriptan (Zomig), and others.

Here's the honest picture with co-codamol (codeine plus paracetamol) and Ceylon Leadwort. The concern on file is that plumbagin, a compound in this plant, may block an enzyme called CYP1A2. But that enzyme isn't the main player for either part of this medicine. Codeine relies mostly on CYP2D6 to turn into its active painkilling form, and paracetamol is handled mainly through other pathways. So the CYP1A2 angle doesn't line up cleanly with how your body processes co-codamol.

On top of that, this is lab-dish evidence only, never reported in real people, so I wouldn't lose sleep over it. Still, because codeine carries real risks (drowsiness, constipation, slowed breathing), check with your pharmacist or doctor before combining them.

Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2E1. Some drugs metabolized by CYP2E1 include acetaminophen, chlorzoxazone (Parafon Forte), ethanol, theophylline, and anesthetics such as enflurane (Ethrane), halothane (Fluothane), isoflurane (Forane), methoxyflurane (Penthrane).

Here's the situation with Co-codamol, which combines codeine with paracetamol (acetaminophen). The piece that connects to Ceylon Leadwort is the paracetamol part. Paracetamol is partly handled by a liver enzyme called CYP2E1, and that pathway can produce a small amount of a toxic byproduct that's normally cleared safely. In lab dish studies only, plumbagin from Ceylon Leadwort appeared to slow down CYP2E1. In theory, that could shift how paracetamol is processed.

The good news: this has never actually been reported in people, and the evidence is just test-tube data, so I wouldn't lose sleep over it. Still, paracetamol and the liver always deserve respect. If you're using Co-codamol regularly, it's worth a quick word with your pharmacist or doctor before adding this supplement, and never exceed your paracetamol dose.
