Why this applies to Gliclazide

Gliclazide falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2C9 (CYP2C9) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2C9. Drugs that are metabolized by CYP2C9 include celecoxib (Celebrex), diclofenac (Voltaren), fluvastatin (Lescol), glipizide (Glucotrol), ibuprofen (Advil, Motrin), irbesartan (Avapro), losartan (Cozaar), phenytoin (Dilantin), piroxicam (Feldene), tamoxifen (Nolvadex), tolbutamide (Tolinase), torsemide (Demadex), and S-warfarin (Coumadin).

Here's the key thing with Gliclazide: your body clears it largely through a liver enzyme called CYP2C9. A compound in Ceylon Leadwort called plumbagin appears to slow that enzyme down, at least in lab dish studies. If that happened in a real person, gliclazide could hang around longer and hit harder, which means your blood sugar might drop too low. Watch for low blood sugar (hypoglycemia) signs like shakiness, sweating, dizziness, confusion, or a racing heart.

I want to be honest about the evidence though. This is only test-tube data, never reported in actual patients, so it's a theoretical concern rather than a proven one. Still, because low blood sugar can be serious, please check with your pharmacist or doctor before pairing the two.

Ceylon leadwort root extract has been shown to both increase and decrease blood glucose levels in animal research. This effect has not yet been demonstrated in humans. Theoretically, Ceylon leadwort might reduce or potentiate the effects of antidiabetes drugs. Monitor blood glucose levels closely. Medication dose adjustments may be necessary. Some antidiabetes drugs include glimepiride (Amaryl), glyburide (Diabeta, Glynase PresTab, Micronase), insulin, pioglitazone (Actos), rosiglitazone (Avandia), and others.

Here's the honest picture with gliclazide (Diamicron) and Ceylon Leadwort. Gliclazide is a sulfonylurea that lowers blood sugar by pushing your pancreas to release more insulin. In animal studies, Ceylon Leadwort root has gone both ways, sometimes lowering blood sugar and sometimes raising it. We just don't have human data, so this is a theoretical concern rather than something we've clearly seen in people.

The thing to watch: if the herb adds to gliclazide's effect, your sugar could drop too low (shakiness, sweating, dizziness, confusion). If it works against it, your readings could creep up instead. If you decide to try it, check your glucose more often and loop in your pharmacist or doctor first. Please don't start, stop, or change your gliclazide on your own.

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts.
