Guselkumab falls under IMMUNOSUPPRESSANTS — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has demonstrated immunosuppressant activity in animal research. Theoretically, Ceylon leadwort might interfere with immunosuppressive therapy. Immunosuppressant drugs include azathioprine (Imuran), basiliximab (Simulect), cyclosporine (Neoral, Sandimmune), daclizumab (Zenapax), muromonab-CD3 (OKT3, Orthoclone OKT3), mycophenolate (CellCept), tacrolimus (FK506, Prograf), sirolimus (Rapamune), prednisone (Deltasone, Orasone), corticosteroids (glucocorticoids), and others.

Here's the gist with Tremfya (guselkumab) and Ceylon Leadwort. Guselkumab is a lab-made antibody that calms an overactive immune signal to clear up psoriasis and ease psoriatic arthritis. The concern on file is that plumbagin, a compound in Ceylon Leadwort, has shown its own immune-affecting activity in test tube and animal studies. In theory, layering it on top of guselkumab could nudge your immune system in unpredictable ways, either adding to the dampening effect or muddying how well your treatment works.

I want to stress this is theoretical only, based on lab data, not real patient cases, so don't panic. Still, since both touch the immune system, it's worth a quick chat with your prescriber or pharmacist before combining them.

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts. Talk to a pharmacist or doctor before using it, and avoid it in pregnancy.

About Guselkumab Injection

What Is Guselkumab Injection Used For?

Brand name: Tremfya

Tremfya (guselkumab) injection is approved to treat adults and children 6 years of age and older who weigh at least 40 kg (about 88 lbs) with moderate-to-severe plaque psoriasis or active psoriatic arthritis. It is also approved for adults with moderately to severely active ulcerative colitis or Crohn's disease.

For plaque psoriasis and psoriatic arthritis, Tremfya is an option when systemic therapy or phototherapy is appropriate. For ulcerative colitis and Crohn's disease, Tremfya is approved for adult patients only — it has not been established as safe and effective for children with those conditions.
