Fomoterol falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2D6 (CYP2D6) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2D6. Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), fluoxetine (Prozac), ondansetron (Zofran), tramadol (Ultram), and others.

Here's the gist: a compound in Ceylon Leadwort called plumbagin can slow down a liver enzyme called CYP2D6, which helps clear formoterol from your body. In theory, blocking that enzyme could let formoterol levels climb a bit higher than usual. Since formoterol is a long-acting beta agonist, more of it floating around could mean stronger beta side effects like a fast or pounding heartbeat, jitteriness or tremor, headache, or muscle cramps.

But keep this in perspective. This concern comes only from lab (test tube) research, not from real people, so it's theoretical at this point. I wouldn't lose sleep over it, but I also wouldn't combine them on your own. Check with your pharmacist or doctor first, especially if you notice any heart racing or shakiness.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2C9 (CYP2C9) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2C9. Drugs that are metabolized by CYP2C9 include celecoxib (Celebrex), diclofenac (Voltaren), fluvastatin (Lescol), glipizide (Glucotrol), ibuprofen (Advil, Motrin), irbesartan (Avapro), losartan (Cozaar), phenytoin (Dilantin), piroxicam (Feldene), tamoxifen (Nolvadex), tolbutamide (Tolinase), torsemide (Demadex), and S-warfarin (Coumadin).

Here's the honest picture: the concern on file is that plumbagin (a compound in Ceylon Leadwort) may slow down a liver enzyme called CYP2C9. The catch is that formoterol doesn't really lean on CYP2C9 to be cleared. Your lungs and body break it down mostly through other routes (like direct chemical processing and a different enzyme family), so this particular interaction isn't a strong match for your inhaler.

On top of that, this finding comes only from lab dish studies, not from people, so it's theoretical at best. I wouldn't expect Ceylon Leadwort to change how well your formoterol opens your airways or to ramp up side effects like jitteriness or a racing heart. Still, since this supplement isn't well studied and can be irritating, please run it by your pharmacist or doctor before adding it.

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts. What Is Formoterol Oral Inhalation Used For?

Brand name: Foradil

Formoterol fumarate inhalation solution is used for the long-term maintenance treatment of airway narrowing (bronchoconstriction) in adults with COPD, including chronic bronchitis and emphysema. It is taken twice daily — morning and evening — as a regular, ongoing treatment.

Important limits: This medication is not for relieving sudden COPD flare-ups or breathlessness in the moment, and it is not approved to treat asthma. A separate short-acting inhaler should always be on hand for sudden symptoms.
