Why this applies to Galantamine Extended Release

Galantamine Extended Release falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2D6 (CYP2D6) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2D6. Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), fluoxetine (Prozac), ondansetron (Zofran), tramadol (Ultram), and others.

Here's the gist for Razadyne ER (galantamine). Your body uses an enzyme called CYP2D6 to help break galantamine down. Plumbagin, a compound in Ceylon Leadwort, has been shown in lab dish studies only to block that same enzyme. If that held true in people, galantamine could clear more slowly and build up a bit, which might make its usual side effects stronger, things like nausea, vomiting, diarrhea, dizziness, a slow heartbeat, or loss of appetite.

I want to be honest, though: this hasn't actually been seen in humans, so it's theoretical right now. Galantamine isn't a prodrug, so blocking the enzyme means more drug, not less. Before adding this supplement, run it by your pharmacist or doctor first.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Here's the gist for Razadyne ER (galantamine): your body clears this medicine partly through a liver enzyme called CYP3A4. A compound in Ceylon Leadwort called plumbagin has been shown to slow that enzyme down, but only in lab dish (in vitro) studies, not in real people. So the concern here is mostly theoretical.

If plumbagin did block that enzyme in your body, galantamine could build up a bit, which might mean stronger cholinergic side effects: nausea, vomiting, diarrhea, dizziness, a slow heartbeat, or feeling faint. Since we don't have human data, I wouldn't panic, but I also wouldn't combine them without a heads-up. Please run it by your pharmacist or doctor before taking Ceylon Leadwort alongside your galantamine.
