Acetaminophen, Chlorpheniramine Maleate, Dextromethorphan HBr falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Devil's Claw interacts with.

Theoretically, devil's claw might increase levels of drugs metabolized by CYP3A4.
In vitro research shows that devil's claw might inhibit CYP3A4, although this has not been reported in humans.

Here's the gist with this combination cold-and-flu product. In test-tube studies, devil's claw appears to slow down a liver enzyme called CYP3A4. Your body uses that enzyme to help clear the dextromethorphan (the cough suppressant) in this medicine. If the enzyme works slower, dextromethorphan could build up a bit, which might mean more drowsiness, dizziness, or stomach upset. The acetaminophen and chlorpheniramine aren't strongly affected this way.

Honestly, this is only a theoretical concern. It's been seen in lab dishes, not in real people, so for most folks taking a short course for a cold it's unlikely to cause trouble. Still, since both can make you sleepy, mention it to your pharmacist before pairing them, especially if you're driving.
