Aspirin, Codeine Phosphate, Phenobarbital falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Fever Bark interacts with.

Theoretically, fever bark might increase the levels and clinical effects of CYP2D6 substrates.
In vitro research suggests that yohimbine, a constituent of fever bark, inhibits CYP2D6 enzyme activity. The effects of fever bark itself are unclear.

This combo product contains codeine, and here's the key point: codeine is a prodrug. Your body needs the liver enzyme CYP2D6 to convert codeine into morphine, which is what actually relieves pain. Fever bark contains a yohimbine-like compound that, in lab studies, can block CYP2D6. If that blocking happened in real life, less codeine would turn into morphine, so you might get weaker pain relief than expected.

I want to be honest about how thin this evidence is. It's only test-tube data, and the real effect of fever bark in people is unknown. The aspirin and phenobarbital parts aren't the concern here. Fever bark itself carries its own risks, so before mixing the two, please check with your pharmacist or doctor.

Theoretically, combining fever bark with antiplatelet or anticoagulant drugs might have additive effects; however, this has not been reported in clinical research.
Research in healthy adults shows that taking yohimbine, a constituent of fever bark, in doses of 8 mg or more, seems to inhibit platelet aggregation in vitro by binding to the alpha-2 adrenoceptor. The effects of fever bark itself are unclear.

This combination product already contains aspirin, which thins the blood by keeping platelets from sticking together. Fever bark contains a yohimbine-like compound that, in lab studies, also seems to make platelets less sticky. So in theory, pairing the two could add up and make minor bleeding a bit easier, think longer bleeding from a cut, more bruising, or stomach upset from the aspirin.

That said, this is a theoretical concern based only on lab work, not on real patients, so the actual risk with fever bark is unclear and likely small. Fever bark also carries its own alkaloids that can affect blood pressure and heart rate, which is reason enough to be cautious. Before mixing them, please check with your pharmacist or doctor.

Fever bark is the bark of the Australian tree Alstonia constricta, used in traditional medicine for fever, diarrhea, and infections. There is very little reliable human research to support these uses, and the bark contains potent alkaloids that may cause serious side effects. It is best avoided unless used under direct medical supervision.
