Theoretically, CYP3A4 inhibitors might increase the levels and clinical effects of yohimbine, a constituent of fever bark.
In vitro and clinical research shows that the fever bark constituent, yohimbine, is metabolized by CYP3A4 enzymes.

Here's the gist: clarithromycin is a strong blocker of a liver enzyme called CYP3A4. Fever bark contains yohimbine, an alkaloid that your body normally clears using that same enzyme. So when clarithromycin slows that pathway down, yohimbine can build up to higher than expected levels in your system.

What does that look like in real life? More of yohimbine's typical effects, like a faster or pounding heartbeat, jitteriness, anxiety, sweating, and changes in blood pressure. I'll be honest, this is a theoretical concern based on how the two work, not on real patient reports, so don't panic. Still, fever bark is a potent product with little safety data. Please check with your pharmacist or doctor before combining them.

Fever bark is the bark of the Australian tree Alstonia constricta, used in traditional medicine for fever, diarrhea, and infections. There is very little reliable human research to support these uses, and the bark contains potent alkaloids that may cause serious side effects. It is best avoided unless used under direct medical supervision.

About Clarithromycin

What Is Clarithromycin Used For?

Brand names: Biaxin, Biaxin XL

Clarithromycin tablets and oral suspension treat a range of mild to moderate bacterial infections in adults and children, including community-acquired pneumonia, acute sinus infections, acute bacterial flare-ups of chronic bronchitis, strep throat and tonsillitis, uncomplicated skin infections, and middle ear infections in children. Tablets are also used in adults to eliminate H. pylori bacteria (in combination with other medicines) to reduce the risk of duodenal ulcer recurrence.

Clarithromycin tablets — but not the extended-release tablet — are also used to treat and prevent disseminated Mycobacterium avium complex (MAC) infections in people with advanced HIV. Clarithromycin extended-release tablets are approved only for acute bacterial exacerbation of chronic bronchitis, acute maxillary sinusitis, and community-acquired pneumonia in adults — not for the other conditions listed above.
