Phenobarbital, Phenytoin falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Goji interacts with.

Theoretically, goji berry might inhibit CYP2C19 and reduce metabolism of CYP2C19 substrates.
In vitro research shows that goji berry tincture and juice inhibit CYP2C19 enzymes. Concomitant use with goji may decrease metabolism and increase levels of CYP2C19 substrates. However, this has not been reported in humans.

Here's the gist: your body uses a liver enzyme called CYP2C19 to help clear phenytoin (and to a degree phenobarbital). In lab tests, goji berry juice and tincture slowed that enzyme down. If that happened in a person, these medicines could clear a little slower and drift higher in the blood. With phenytoin especially, which has a narrow safety window, even small bumps can tip you into side effects like dizziness, unsteady walking, slurred speech, double vision, or drowsiness.

The important caveat: this is only theoretical and comes from test-tube work, never reported in actual patients. A few goji berries as food are very unlikely to matter. Concentrated supplements are the bigger question. Before adding goji capsules or juice, check with your pharmacist or doctor, and never adjust your seizure medicine on your own.

Additionally, multiple case reports suggest that goji berry concentrated tea and juice inhibit the metabolism of warfarin, a CYP2C9 substrate. Concomitant use with goji may decrease metabolism and increase levels of CYP2C9 substrates.

Here's the gist for your seizure medicine. Goji berry may slow down a liver enzyme called CYP2C9, which is one of the main workers that breaks down phenytoin. If that enzyme is partly blocked, phenytoin can build up in your blood. With phenytoin that matters a lot, because it has a narrow safe range. Too much can cause dizziness, unsteady walking, slurred speech, jerky eye movements, drowsiness, or confusion.

Now, the honest part: this is mostly theoretical and based on lab tests plus warfarin case reports, not strong phenytoin studies. Eating a few goji berries probably won't matter much. Concentrated goji juice, tea, or supplements are the bigger concern. Please check with your pharmacist or doctor before adding them, since your dose is carefully balanced.
