Here's the honest picture: green tea and metformin don't have a strong, well-proven clash. The concern is mostly theoretical. Green tea (and its caffeine) has been reported to nudge blood sugar in either direction, and the studies frankly disagree with each other, so the effect is small and unpredictable.

Metformin works in your liver and gut rather than relying on one enzyme to activate it, so green tea isn't going to dramatically change how it works. If anything, you might just notice your blood sugar readings wobble a little. The simple move is to keep an eye on your home glucose numbers, especially if you start drinking large amounts or take concentrated green tea extract pills. Normal cups of tea are unlikely to matter. Mention any changes to your pharmacist or doctor.

Theoretically, metformin might increase the levels and adverse effects of caffeine.
Green tea contains caffeine. Animal research suggests that metformin can reduce caffeine metabolism. Theoretically, concomitant use can increase caffeine serum concentrations and the risk of caffeine adverse effects.

Here's a twist worth knowing: in this pairing it's actually the caffeine in green tea that may be affected, not your metformin. Some animal research hints that metformin might slow how your body clears caffeine, so the caffeine could hang around a bit longer than usual. Your blood sugar control from metformin shouldn't change because of green tea.

If caffeine does build up, you might notice the usual jittery stuff: a racing heart, feeling wired, trouble sleeping, or an upset stomach. Honestly though, this is only a theory based on lab and animal work, so for most people drinking normal amounts of green tea it's nothing to lose sleep over. If you feel extra wired, ease back on the caffeine and mention it to your pharmacist.

Metformin hydrochloride tablets and oral solution are approved for adults and children 10 years and older. Metformin hydrochloride extended-release tablets — including brands such as Fortamet, Glumetza, and Metformin Hydrochloride Extended Release — are approved for adults only.
