Brexpiprazole falls under ANTIPSYCHOTIC DRUGS — the group Khat interacts with.

Khat may affect response to antipsychotic treatment. In a large retrospective study of adults with newly diagnosed schizophrenia, chewing khat was associated with a decreased retention rate with the initial antipsychotic agent when compared with non-chewers, and lack of efficacy was the most commonly reported reason for switching antipsychotic agents among khat users. Until more is known, avoid khat in patients taking antipsychotic agents.

Here's the tension with these two: Rexulti (brexpiprazole) is working to calm and balance the brain chemicals that drive symptoms of schizophrenia and depression. Khat does the opposite kind of thing. Its main chemical, cathinone, acts a lot like an amphetamine and floods your brain with dopamine and norepinephrine. That can push right back against what your medication is trying to do.

In a large real-world study of people newly diagnosed with schizophrenia, those who chewed khat were more likely to have their antipsychotic stopped or switched, usually because it just wasn't working well enough. So with brexpiprazole, the worry is weaker symptom control, things like returning agitation, mood swings, or unusual thoughts. Beyond that, khat is illegal in the U.S. and hard on the heart. Please talk with your doctor or pharmacist before mixing anything like this.

What Is Brexpiprazole Used For?

Brand name: Rexulti

Brexpiprazole tablets are approved for two uses in adults: as an add-on (adjunctive) treatment alongside antidepressants for major depressive disorder (MDD), and as a treatment for schizophrenia. These uses apply to both the generic brexpiprazole tablets and Rexulti.

Rexulti has one additional approved use: treating agitation associated with dementia due to Alzheimer's disease in adults. Rexulti is also approved for schizophrenia in adolescents ages 13 and older. Note that Rexulti is not approved for dementia-related psychosis without agitation from Alzheimer's disease, and it is not meant to be used on an as-needed basis for Alzheimer's-related agitation.
