CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES

Abrocitinib falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Melatonin interacts with.
Melatonin is metabolized in the liver primarily by the CYP2C19 and CYP1A2 enzymes. Theoretically, combined administration of melatonin with certain drugs metabolized by the CYP2C19 enzyme may reduce the metabolism of these drugs, resulting in increased serum levels. Conversely, some drugs metabolized by CYP2C19 may inhibit the metabolism of melatonin, resulting in increased serum levels of melatonin. Until more is known, use melatonin cautiously in patients taking drugs metabolized by these enzymes.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the gist for Cibinqo (abrocitinib): your liver uses an enzyme called CYP2C19 to help break it down, and melatonin leans on that same enzyme. On paper, taking melatonin alongside abrocitinib could slow how fast your body clears the medication, which might nudge its levels up a bit. With a JAK inhibitor like this, higher levels could mean a greater chance of side effects such as nausea, headaches, acne, or a higher risk of infection.
I want to be honest, though: this is a theoretical concern based on how the drugs work, not strong real-world data. For most people it's likely minor. Still, since abrocitinib affects your immune system, it's worth a quick chat with your pharmacist or doctor before adding melatonin.

ANTICOAGULANT/ANTIPLATELET DRUGS

There are isolated case reports of minor bleeding and decreased prothrombin activity in people taking melatonin with warfarin (Coumadin). The mechanism, if any, of this interaction is unknown. Taking melatonin orally seems to decrease coagulation activity within one hour of dosing in healthy men.

Here's the thing to keep in mind with this pairing. Melatonin itself may have a mild blood-thinning effect, and abrocitinib (Cibinqo) carries its own small bleeding-related risk because it can lower your platelet count, especially early in treatment. So the concern is that stacking the two could nudge your bleeding risk a little higher. You might notice things like easier bruising, nosebleeds, or bleeding gums.
I'll be honest, this interaction is mostly theoretical. The blood-thinning signal for melatonin comes from a few scattered case reports and small studies, not strong proof. Occasional melatonin for sleep is unlikely to cause trouble, but since abrocitinib already needs blood monitoring, it's worth a quick chat with your pharmacist or doctor before combining them.

Melatonin can stimulate immune function. Theoretically, melatonin might interfere with immunosuppressive therapy.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the idea behind this pairing. Abrocitinib (Cibinqo) works by quieting down part of your immune system to calm the inflammation that drives bad eczema. Melatonin, on the other hand, can gently nudge the immune system to be more active. So in theory they pull in opposite directions, and melatonin could chip away at how well abrocitinib controls your skin.
I want to be honest about the evidence: this is mostly theoretical and not something we see clearly in real life. If melatonin did blunt your treatment, you might notice your itching, redness, or rashes creeping back. That's not a reason to panic, but it's worth a quick chat with your pharmacist or dermatologist before starting melatonin regularly, rather than stopping or changing anything on your own.

What Is Abrocitinib Used For?
Brand name: Cibinqo
Cibinqo (abrocitinib) is used to treat moderate-to-severe atopic dermatitis (eczema) in adults and children 12 years of age and older. It is specifically for people whose eczema has not been adequately controlled with other systemic (whole-body) treatments, including biologics, or when those treatments are not a safe option.
Cibinqo is not recommended to be used at the same time as other JAK inhibitors, biologic immunomodulators, or other immunosuppressants (medicines that reduce immune system activity).
