Why this applies to Estradiol, Norgestimate
Estradiol, Norgestimate falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Melatonin interacts with.
Melatonin is metabolized in the liver primarily by the CYP2C19 and CYP1A2 enzymes. Theoretically, combined administration of melatonin with drugs metabolized by the CYP1A2 enzyme might reduce the metabolism of these drugs, resulting in increased serum levels. Conversely, some drugs metabolized by CYP1A2 may inhibit the metabolism of melatonin, resulting in increased serum levels of melatonin. Until more is known, use melatonin cautiously in patients taking drugs metabolized by these enzymes.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the gist with Ortho-Prefest (your estradiol plus norgestimate combo) and melatonin. The estrogen part is partly handled by a liver enzyme called CYP1A2, which melatonin can theoretically slow down a touch. If that happens, your estrogen levels could nudge up slightly, which might mean a bit more of the usual estrogen-related effects like breast tenderness, bloating, headaches, or nausea.
I want to be honest, though: this is a theoretical concern based on how these compounds behave in the lab, not something well documented in real patients. For most people taking short-term melatonin for sleep, it's unlikely to cause a noticeable problem. Still, if you notice new tenderness or headaches, or you just want peace of mind, run it by your pharmacist or doctor before combining them.
