Melatonin and Glipizide, Metformin

Here is the interaction we found between these two, with the evidence behind it.

Moderate

ANTIDIABETES DRUGS

Why this applies to Glipizide, Metformin

Glipizide, Metformin falls under ANTIDIABETES DRUGS — the group Melatonin interacts with.

Theoretically, taking melatonin with antidiabetes drugs might increase the risk of hypoglycemia.
Some clinical research shows that melatonin reduces levels of fasting blood glucose and improves glycemic control. However, other research suggests that melatonin might impair glucose utilization and increase insulin resistance, while other research has found no effect on glucose levels. Until more is known, use melatonin cautiously in combination with antidiabetes drugs.

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Metaglip combines two diabetes medicines: glipizide, which pushes your pancreas to release more insulin, and metformin, which lowers the sugar your liver makes. Because glipizide can drive blood sugar down on its own, it's the one that can cause true lows. The concern here is that melatonin's effect on blood sugar isn't fully predictable. Some studies show it nudges sugar a little lower, others suggest it may actually raise sugar or worsen insulin resistance, and some show no change at all.

This is mostly theoretical, so I wouldn't lose sleep over it. Still, if you add melatonin, keep an eye out for signs of a low (shakiness, sweating, dizziness, confusion). Check your readings and chat with your pharmacist or doctor before starting.

From our pharmacist’s overview on the Glipizide, Metformin page

“Glipizide and metformin (Metaglip) already work hard to pull blood sugar down, which is exactly why this list of 307 ingredients deserves genuine vigilance rather than a shrug. Most of them, 239, can lower glucose on their own, and stacked on this pill the combined effect can tip into true hypoglycemia.”

About Melatonin

What is Melatonin?

Melatonin
  • Trouble falling asleep (insomnia)
  • Jet lag
  • Delayed sleep phase syndrome
  • Non-24-hour sleep-wake disorder
  • Shift-work sleep problems
  • Sleep problems in some children with conditions like autism or ADHD

Melatonin is a hormone your body makes naturally to help control your sleep-wake cycle, and the supplement form is widely used to help with sleep timing problems and jet lag. The evidence is strongest for certain circadian (body-clock) disorders and is more mixed for everyday insomnia. It is generally well tolerated for short-term use, but talk with your pharmacist or doctor before starting, especially for children or during pregnancy.
