Melatonin + Topiramate
Topiramate brand names: Topamax, Trokendi XR, Qudexy XR, Eprontia.
Topiramate falls into 2 drug categories that Melatonin can interact with; the most serious is rated moderate. Theoretically, taking melatonin might increase the sedative effects of CNS depressants.

These two interact in 2 distinct ways. Each one is explained separately below.
Why Are There 2 Interactions?
Melatonin interacts with whole categories of medication. Topiramate falls into 2 of those categories, so it can interact with Melatonin in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CNS DEPRESSANTS
1 of 2Why this applies to Topiramate
Topiramate falls under CNS DEPRESSANTS — the group Melatonin interacts with.
Melatonin has sedative effects. Theoretically, concomitant use of melatonin with alcohol, benzodiazepines, or other sedative drugs might cause additive sedation.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version: both melatonin and topiramate can make you feel drowsy on their own, so taking them together may stack that sleepy effect. With topiramate, people often notice tiredness, slowed thinking, trouble concentrating, or feeling a bit foggy, and adding melatonin could make those feelings a little stronger, especially at night.
I want to be honest about how solid this is: it's a theoretical concern based on how the two work, not something proven to cause problems in most people. This isn't about topiramate building up in your body, it's just the two calming effects overlapping. If you decide to try melatonin, see how you feel before driving or doing anything that needs sharp focus, and check with your pharmacist or doctor first.

ANTICONVULSANTS
2 of 2Topiramate also falls under ANTICONVULSANTS.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the situation with melatonin and topiramate. The concern isn't really about your body breaking the medicine down differently. Instead, some clinical research suggests melatonin itself may make seizures happen more often in certain people, especially children who have neurological conditions. If that happened, it could work against what topiramate is doing to keep seizures under control, or undercut its benefit for migraine prevention.
I want to be honest that this is a possible, not certain, interaction, and a good chunk of it is theoretical or seen in specific groups. Many adults use melatonin short-term without trouble. Still, because seizure control is so important, please check with your pharmacist or doctor before adding melatonin rather than starting it on your own.
What These Ratings Mean
Every interaction above is graded three ways — here’s the key to each label.
Clinically significant. Avoid the combination, or use only under close professional supervision.
Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.
Generally not serious. Be aware of it and mention it to your pharmacist.
- LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
- ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
- PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
- UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
- AHigh-quality meta-analysis (quantitative systematic review).
- BCase-control study.
- CConsensus.

About Melatonin
What is Melatonin?
- Trouble falling asleep (insomnia)
- Jet lag
- Delayed sleep phase syndrome
- Non-24-hour sleep-wake disorder
- Shift-work sleep problems
- Sleep problems in some children with conditions like autism or ADHD
Melatonin is a hormone your body makes naturally to help control your sleep-wake cycle, and the supplement form is widely used to help with sleep timing problems and jet lag. The evidence is strongest for certain circadian (body-clock) disorders and is more mixed for everyday insomnia. It is generally well tolerated for short-term use, but talk with your pharmacist or doctor before starting, especially for children or during pregnancy.
About Topiramate
What Is Topiramate Used For?
Brand names: Topamax, Trokendi XR, Qudexy XR, Eprontia
-
Topiramate is used alone or with other medications to treat certain types of seizures including primary generalized tonic-clonic seizures (formerly known as a grand mal seizure; seizure that involves the entire body) and partial onset seizures (seizures that involve only one part of the brain). Topiramate is also used with other medications to control seizures in people who have Lennox-Gastaut syndrome (a disorder that causes seizures and developmental delays). Topiramate is also used to prevent migraine headaches but not to relieve the pain of migraine headaches when they occur. Topiramate is in a class of medications called anticonvulsants. It works by decreasing abnormal excitement in the brain.

Topiramate Is Linked to Lower Levels of One Nutrient
Natural Medicines records Topiramate as a medicine that can lower these. This is not an interaction with Melatonin — it is a separate effect of the medicine, and it is a reason some people are advised to supplement rather than to avoid one:
