Abiraterone Acetate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Milk Thistle interacts with.

It is unclear if milk thistle inhibits CYP3A4; research is conflicting.
While laboratory research shows conflicting results, pharmacokinetic research shows that taking milk thistle extract 420-1350 mg daily does not significantly affect the metabolism of the CYP3A4 substrates irinotecan, midazolam, or indinavir. However, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP3A4 substrates, including gefitinib, sorafenib, doxorubicin, and vincristine.

Here's the short version on pairing milk thistle with your abiraterone (Yonsa). Your body clears abiraterone partly through a liver enzyme called CYP3A4. The thought is that milk thistle might slow that enzyme down, which in theory could let abiraterone build up a bit and nudge up side effects like fatigue, high blood pressure, swelling in your legs, low potassium, or changes in your liver labs.

The good news: this is mostly lab-bench speculation, and human studies haven't shown milk thistle meaningfully changing CYP3A4 drugs. The interaction is rated minor and unlikely. Still, since abiraterone is a serious cancer medicine, I'd run any milk thistle plans past your oncologist or pharmacist first so they can keep an eye on your labs.

Milk thistle is a popular herbal supplement most often used for liver health, and its main active component is a group of compounds called silymarin. While it is generally well tolerated, the scientific evidence that it meaningfully improves liver disease or other conditions is mixed and far from conclusive. About Abiraterone

What Is Abiraterone Used For?

Brand names: Zytiga, Yonsa

Abiraterone acetate tablets are used to treat two types of advanced prostate cancer: metastatic castration-resistant prostate cancer (CRPC — cancer that has spread and keeps growing despite low testosterone levels) and metastatic high-risk castration-sensitive prostate cancer (CSPC — cancer that has spread but still responds to hormone-lowering treatment). It is always taken together with a corticosteroid — most products pair with prednisone, while Yonsa is specifically indicated with methylprednisolone.

All products — including Abiraterone, Abiraterone Acetate, Abirtega, Yonsa, and Zytiga — are taken by mouth as a tablet. Yonsa is approved only for metastatic CRPC, while the other products (Abiraterone, Abiraterone Acetate, Abirtega, and Zytiga) are approved for both metastatic CRPC and metastatic high-risk CSPC.
