Acetaminophen, Butalbital, Codeine falls under GLUCURONIDATED DRUGS — the group Milk Thistle interacts with.

Theoretically, milk thistle might affect the clearance of drugs that undergo glucuronidation.
Laboratory research shows that milk thistle constituents inhibit uridine diphosphoglucuronosyl transferase (UGT), the major phase 2 enzyme that is responsible for glucuronidation. Theoretically, this could decrease the clearance and increase levels of glucuronidated drugs. Other laboratory research suggests that a milk thistle extract of silymarin might inhibit beta-glucuronidase, although the significance of this effect is unclear.

Here's the gist of it. This combo pill (acetaminophen, butalbital, and codeine) is partly cleared by your liver through a process called glucuronidation. In test tube studies, milk thistle's silymarin seems to slow that enzyme down a bit. In theory, that could let some of these drugs hang around a little longer than usual, which might add to side effects like drowsiness, dizziness, constipation, or feeling foggy.

That said, this is lab-bench data, not real people, so I wouldn't lose sleep over it. The bigger thing to watch is that codeine and butalbital are already sedating, and acetaminophen is hard on the liver in large amounts. Since you're using milk thistle for liver support anyway, just loop in your pharmacist or doctor before combining them so they can keep an eye on dosing.
