Theoretically, milk thistle might affect the clearance of drugs that undergo glucuronidation.
Laboratory research shows that milk thistle constituents inhibit uridine diphosphoglucuronosyl transferase (UGT), the major phase 2 enzyme that is responsible for glucuronidation. Theoretically, this could decrease the clearance and increase levels of glucuronidated drugs. Other laboratory research suggests that a milk thistle extract of silymarin might inhibit beta-glucuronidase, although the significance of this effect is unclear.

This combo cold product (the brand Coricidin II) packs acetaminophen, chlorpheniramine, and dextromethorphan together. Acetaminophen in particular leans on a liver process called glucuronidation to be cleared from your body. The concern here is that milk thistle's silymarin compounds have, in lab and animal studies only, slowed that same enzyme system. In theory that could let the drugs hang around a bit longer, nudging up effects like drowsiness, dizziness, or dry mouth from the chlorpheniramine.

The honest truth is this is a theoretical concern with weak evidence, and I would not lose sleep over occasional use. Still, since acetaminophen can stress the liver and milk thistle is often taken for liver reasons, it is worth a quick chat with your pharmacist before pairing them regularly.

It is unclear if milk thistle inhibits CYP3A4; research is conflicting.
While laboratory research shows conflicting results, pharmacokinetic research shows that taking milk thistle extract 420-1350 mg daily does not significantly affect the metabolism of the CYP3A4 substrates irinotecan, midazolam, or indinavir. However, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP3A4 substrates, including gefitinib, sorafenib, doxorubicin, and vincristine.

Here's the bottom line on pairing milk thistle with this Coricidin cold and flu combo (acetaminophen, chlorpheniramine, and dextromethorphan). The theory is that milk thistle might slow down a liver enzyme called CYP3A4, which helps clear part of these ingredients (especially dextromethorphan). If that happened, levels could drift up a bit, and you might feel more drowsy, dizzy, or foggy than usual.

In real life, though, this looks very unlikely to matter. The best human studies show milk thistle at normal doses does not meaningfully change CYP3A4 medicines, and the worry here comes mostly from lab tests and a few cancer-drug case reports, not from cold remedies. I wouldn't lose sleep over it. Still, if you take milk thistle daily, mention it to your pharmacist so they can keep an eye on things.
