ADO-Trastuzumab Emtansine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Milk Thistle interacts with.
While laboratory research shows conflicting results, pharmacokinetic research shows that taking milk thistle extract 420-1350 mg daily does not significantly affect the metabolism of the CYP3A4 substrates irinotecan, midazolam, or indinavir. However, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP3A4 substrates, including gefitinib, sorafenib, doxorubicin, and vincristine.
Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..
Here's the gist for Kadcyla (ado-trastuzumab emtansine). Part of this drug, the emtansine (DM1) piece that actually kills cancer cells, is broken down by a liver enzyme called CYP3A4. The theory is that if milk thistle slows that enzyme down, more of the active toxin could hang around and push up side effects like low blood counts, low platelets (more bruising or bleeding), nausea, fatigue, and liver enzyme changes.
That said, the evidence here is weak. Lab results conflict, and human studies haven't shown milk thistle meaningfully changing other CYP3A4 drugs. A handful of case reports flagged extra toxicity with cancer meds, so caution makes sense. Since this is a serious chemotherapy, please loop in your oncology team or pharmacist before adding milk thistle.

Kadcyla is used by itself (as a single agent) for HER2-positive breast cancer in two situations. Your tumor must test positive for HER2 using an FDA-authorized test.
- Metastatic breast cancer: for people who already received trastuzumab and a taxane, and who either had treatment for metastatic disease or whose cancer returned during or within six months of finishing adjuvant (after-surgery) therapy.
- Early breast cancer: as adjuvant treatment for people who still have residual invasive disease after neoadjuvant (pre-surgery) taxane and trastuzumab-based treatment.
