Why this applies to Alvimopan

Alvimopan falls under P-GLYCOPROTEIN SUBSTRATES — the group Milk Thistle interacts with.

Theoretically, milk thistle might increase the absorption of P-glycoprotein substrates. However, this effect does not seem to be clinically significant.
In vitro research shows that milk thistle can inhibit P-glycoprotein activity and 1 case report from the World Health Organization (WHO) adverse drug reaction database describes increased abdominal pain in a patient taking milk thistle and the cancer medication vincristine, a P-glycoprotein substrate, though this patient was also taking methotrexate. However, a small pharmacokinetic study in healthy volunteers shows that taking milk thistle (Enzymatic Therapy Inc.) 900 mg, standardized to 80% silymarin, in 3 divided doses daily for 14 days does not affect absorption of digoxin, a P-glycoprotein substrate.

Here's the short version: alvimopan (Entereg) is carried by a transporter in your gut called P-glycoprotein, and milk thistle might nudge that transporter to let a little more drug get absorbed. In theory, that could mean slightly higher alvimopan levels. The good news is that this concern is very weak. The evidence comes mostly from lab dishes, and a careful human study with another P-gp drug (digoxin) showed milk thistle didn't really change its absorption.

So for most people, this is unlikely to cause a real problem. Alvimopan is also used only short-term right after bowel surgery, usually in the hospital. Still, since it's used in that recovery window, it's worth telling your surgical team or pharmacist if you take milk thistle, just so they have the full picture.

What Is Alvimopan Used For?

Brand name: Entereg

Alvimopan capsules are used in hospitals to help the digestive tract recover more quickly after surgery that involves removing part of the bowel (partial bowel resection with primary anastomosis — meaning the remaining ends are reconnected). After this type of surgery, the bowel often temporarily stops working, a condition called postoperative ileus.

This medication is only given during a short hospital stay and is not for long-term or home use.
