Why this applies to Aspirin, Omeprazole

Aspirin, Omeprazole falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Milk Thistle interacts with.

It is unclear if milk thistle inhibits CYP2C9; research is conflicting.
In vitro research suggests that milk thistle might inhibit CYP2C9. Additionally, 3 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP2C9 substrates, including imatinib and capecitabine. However, contradictory clinical research shows that milk thistle extract does not inhibit CYP2C9 or significantly affect levels of the CYP2C9 substrate tolbutamide. Differences in results could be due to differences in dosages or formulations utilized.

Here's the gist with Yosprala (the aspirin plus omeprazole combo) and milk thistle. One enzyme in your liver, called CYP2C9, helps clear part of this medication. In test-tube studies, milk thistle might slow that enzyme down a bit. If it did, the worry would be slightly higher levels of the medicine, which for aspirin could mean a touch more stomach irritation or easier bruising and bleeding.

That said, the real-world evidence here is weak and honestly conflicting. Some human studies found milk thistle didn't block CYP2C9 at all. The interaction is rated minor and unlikely, so for most people it's not a big concern. Still, if you take both regularly, mention it to your pharmacist or doctor so they can keep an eye on you.

It is unclear if milk thistle inhibits CYP3A4; research is conflicting.
While laboratory research shows conflicting results, pharmacokinetic research shows that taking milk thistle extract 420-1350 mg daily does not significantly affect the metabolism of the CYP3A4 substrates irinotecan, midazolam, or indinavir. However, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP3A4 substrates, including gefitinib, sorafenib, doxorubicin, and vincristine.

Here's the short version: the theory is that milk thistle might slow down a liver enzyme called CYP3A4, which could let certain drugs build up. But honestly, the evidence here is weak and conflicting. Human studies using fairly large doses of milk thistle (up to about 1,350 mg a day) showed no real change in how the body handles CYP3A4 medications.

For your Yosprala (aspirin and omeprazole) specifically, this isn't a combo I'd lose sleep over. Omeprazole is only minimally affected by this pathway, and aspirin doesn't really rely on CYP3A4 at all. If anything theoretical did happen, you'd watch for stomach upset or easy bruising. Even so, it's smart to let your pharmacist know you take milk thistle so they can keep an eye on the whole picture.
