Benzhydrocodone, Acetaminophen falls under GLUCURONIDATED DRUGS — the group Milk Thistle interacts with.

Theoretically, milk thistle might affect the clearance of drugs that undergo glucuronidation.
Laboratory research shows that milk thistle constituents inhibit uridine diphosphoglucuronosyl transferase (UGT), the major phase 2 enzyme that is responsible for glucuronidation. Theoretically, this could decrease the clearance and increase levels of glucuronidated drugs. Other laboratory research suggests that a milk thistle extract of silymarin might inhibit beta-glucuronidase, although the significance of this effect is unclear.

Here's the short version on combining milk thistle with Apadaz (benzhydrocodone and acetaminophen). The acetaminophen part of this medicine is partly cleared from your body through a process called glucuronidation. In test tube studies, milk thistle compounds appear to slow the enzyme that handles this step. The thinking is that, in theory, this could cause acetaminophen to clear a bit slower and build up slightly.

I want to be honest about how weak this evidence is. This comes from lab and animal research only, not real people, so we don't actually see this play out in practice. Still, if levels did rise, you could in theory get more drowsiness, constipation, or strain on the liver. Acetaminophen and your liver are worth respecting, so check with your pharmacist or doctor before pairing the two.

It is unclear if milk thistle inhibits CYP3A4; research is conflicting.
While laboratory research shows conflicting results, pharmacokinetic research shows that taking milk thistle extract 420-1350 mg daily does not significantly affect the metabolism of the CYP3A4 substrates irinotecan, midazolam, or indinavir. However, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP3A4 substrates, including gefitinib, sorafenib, doxorubicin, and vincristine.

Here's the practical picture. Apadaz pairs benzhydrocodone, which your body converts into hydrocodone (the actual painkiller), with acetaminophen. Some of this processing runs through a liver enzyme called CYP3A4. The thinking is that milk thistle might slow that enzyme down, which in theory could let the opioid linger a bit longer and nudge up effects like drowsiness, constipation, nausea, or slowed breathing.

That said, the evidence here is thin. It mostly comes from lab and animal studies, and human studies actually showed milk thistle had little real effect on similar drugs. So this is unlikely to cause trouble for most people. Still, because we're talking about an opioid, it's worth a quick word with your pharmacist or doctor before combining them.

What Is Benzhydrocodone and Acetaminophen Used For?

Brand name: Apadaz

Apadaz is indicated for the short-term management of acute pain — up to 14 days — that is severe enough to require an opioid and for which non-opioid treatments have not worked or are not expected to work. It combines benzhydrocodone (a prodrug that converts to the opioid hydrocodone) with acetaminophen.

Because of serious risks including addiction, abuse, and misuse, Apadaz is reserved for patients who have not tolerated or have not gotten adequate relief from alternative pain treatments. It should not be used for an extended period of time unless the pain remains severe enough to require an opioid and alternatives remain inadequate.
