Brincidofovir falls under ORGANIC ANION-TRANSPORTING POLYPEPTIDE SUBSTRATES (OATP) — the group Milk Thistle interacts with.

Milk thistle may inhibit one form of OATP, OATP-B1, which could reduce the bioavailability and clinical effects of OATP-B1 substrates.
In vitro research shows that milk thistle inhibits OATP-B1. Two case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are OATP substrates, including sorafenib and methotrexate. OATPs are expressed in the small intestine and liver and are responsible for the uptake of drugs and other compounds into the body. Inhibition of OATP may reduce the bioavailability of oral drugs that are substrates of OATP.

Here's the gist with brincidofovir (Tembexa), the antiviral used for smallpox. Your body relies on transporter proteins called OATPs to help pull certain drugs into the bloodstream and liver. In lab studies, milk thistle's main compound (silymarin) can block one of these transporters, OATP-B1. The theoretical worry is that this could lower how much brincidofovir actually gets absorbed, which might mean less of the antiviral effect you're counting on.

I want to be honest about the strength here: this is based on test-tube data, not human studies with this exact drug, so it's labeled minor and only possible. Still, because brincidofovir is used for something serious, don't add milk thistle on your own. Run it by your doctor or pharmacist first.
